Role Scope  

  • Provide excellent clinical patient care as a practice nurse – reactive (e.g. dressings, investigations, Nebulisers, clinical assessments) and planned care (e.g. care plans, health assessments, home visits, reviews, vaccinations)
  • Provide care coordination for chronic disease patients – review care plans and audit/implement chronic disease pathways
  • Provide telehealth consults for patients (working with GPs in the practice) – video, asynchronous messaging, by phone
  • Work with GPs, allied health clinicians, health coaches, practice staff, and occasionally with specialists, to deliver the best clinical care for patients and achieve the Osana objectives
  • Deliver the Osana model of care (e.g. team-based care, slow medicine, population health management, telehealth and home visits)

SKILLS:

  • Highly organised, efficient worker, keen to learn
  • Excellent interpersonal skills, effective written and verbal communications
  • IT savy and proficient in online/mobile Apps
  • Can-do attitude and able to own and solve problems
  • Analytical skills – comfortable with data and performance measurement

EXPERIENCE and REQUIREMENTS:

  • Primary care or chronic disease management experience
  • AHPRA registered (or equivalent) with no conditions to clinical practice
  • Professional indemnity and/or active CPR certification

TERMS:

  • Remuneration base salary plus annual bonus, depending on experience
  • Standard employment conditions – 9.5% super, 20 days annual and 10 days personal leave
  • Hours 8.30am – 5.30pm weekdays with occasional evening/Saturday if business event
  • Sydney based
