What is the difference?

Health assistants are a core part of your care team and actively engage in your health plan to support you to achieve your goals and stay healthy.
Their mission is to help care for you within the Osana family.

What can I expect from my health assistant?

Your Health Assistant is dedicated to supporting you at Osana. They are here to guide you every step towards better health.
They will maintain regular contact with you and act as your first point of call for any concerns, issues or referrals.
You can reach out to them directly for any assistance, including scheduling forward appointments, following up with medical certificates or repeat prescriptions, or accessing services in the community.
You can rely on them to motivate and support you on your health journey. They will keep you up to date with your progress and informed of any services relevant for you including classes and programs

How can I access my Health Assistant?

Ensure you save their mobile number and email on your phone for easy access so you can call, text or email them for questions or concerns. Please note that this is within Osana working hours; however, we will endeavour to prioritise your requests as soon as we receive them.
We encourage you to reach out to your Health Assistant if you have any queries or wish to learn more about vaccinations, surveys, Osana membership, services or your health plan.

What does my health assistant do behind the scenes?

They collate results for your surveys, tests and referrals to ensure we document your progress and provide the care that you need.
On a monthly or quarterly basis, depending on your risk factors, they check in with you to see how your health plan is tracking.
They schedule all your sessions at Osana in the coming 12 months to ensure you do not miss any reviews with your care team.
And your Health Assistant will remind you when assessments are due and suggest services to achieve the best result possible for you.
